Meals on Wheels supports the idea that pets provide love and companionship and improve quality of life for their owners. Therefore, we deliver monthly supplies of cat and dog food to our meal recipients who need assistance with meals for their pets. Your donation of cat or dog food or dollars to support this program is greatly appreciated. The gratifying aspect of our program is that we know how much comfort a pet can bring and we are glad to know that we can continue to keep both our clients AND their beloved pets fed. Once the application is complete, it can be mailed, faxed or dropped off at our office. Application Process Anyone may recommend a person to be considered for receiving Meals on Wheels. Most of our referrals come from family members, home health agencies, hospitals, social services, or the clients themselves.
Case Management Our first priority is always providing a nourishing noon meal, but the case managers do much more than assess clients concerning meals. In addition, because many seniors are unaware of how to access community resources, Meals on Wheels offers a limited program of case management that connects clients with needed services and helps during a crisis. Through case management services, clients receive an assessment of their needs. A care plan is developed which includes a variety of options that allow our clients to remain in his or her own home. The case manger will monitor the care plan and offer support for the client and his or her family members. Our case managers really do perform miracles that greatly improve the quality of our clients lives and allow them to live in their own homes for as long as possible.
