Once the application is complete, it can be mailed, faxed or dropped off at our office.

Application Process
Anyone may recommend a person to be considered for receiving Meals on Wheels. Most of our referrals come from family members, home health agencies, hospitals, social services, or the clients themselves.

• Once we receive the application, our client services staff will call the applicant, referral source or designated person within 24-48 hours to determine basic eligibility.
• If a reduced fee is needed, a case manager will be assigned to set up a home visit.
• Case managers try to set appointments with potential clients within three working days of receiving the application.
• If a fee reduction is not needed, a completed application must be received two business days before meal service is needed

Case Management
Our first priority is always providing a nourishing noon meal, but the case managers do much more than assess clients concerning meals.  In addition, because many seniors are unaware of how to access community resources, Meals on Wheels offers a limited program of case management that connects clients with needed services and helps during a crisis.

Through case management services, clients receive an assessment of their needs. A care plan is developed which includes a variety of options that allow our clients to remain in his or her own home. The case manger will monitor the care plan and offer support for the client and his or her family members.

Our case managers really do perform miracles that greatly improve the quality of our clients lives and allow them to live in their own homes for as long as possible.
